To make a new claim, please click on the link for your Local Authority, and follow the instructions. You will need to complete all relevant sections otherwise the claim form will not be accepted. If you have been self employed for more than a year please provide details for the last financial year. If you have been self employed for less than a year please provide details from the date your business started to date. Please only list business expenses and ensure that the amounts declared cover the period for which you are completing the form.
